The USPS Form 1583
What is it?
Form 1583 is called Application for Delivery of Mail Through an Agent. It's the document that gives us permission to receive your mail for you.
Normally, USPS delivers your mail to you. When you use a mailbox service, USPS is handing your mail to a company instead — and it won't do that unless you've authorized it in writing. That authorization is Form 1583. It names you, names us as your agent, and confirms your identity with two forms of ID.
It's one page, it takes about five minutes, and every private mailbox holder in the United States has one — with every mail service in the country, not just ours. We handle it from start to finish and guide you through every step.
Authorization
Every adult receiving mail needs their own form and two forms of ID.
It does NOT forward your mail
This form only authorizes us to receive your mail — the change of address is separate.
Video call or notary
USPS requires we witness your signature. A two-minute video call does it or an online notary if you prefer.
Your privacy
La Conexion never shares your information with anyone except for USPS.
Who needs to complete a form?
Every adult who will receive mail at your mailbox — including your spouse or partner if they're listed as a secondary box holder. Each person completes their own form with their own two IDs. Children under 18 are listed on a parent's form.
What ID do I need?
Two documents per adult: one photo ID and one document showing your home address.
Photo ID — any one of these:
Passport (U.S. or foreign) · U.S. State/Territory/Tribal Driver's or Nondriver's ID Card · U.S. Permanent Resident Card · Certificate of Naturalization · Matrícula Consular · Uniformed Service ID · U.S. Access Card · U.S. University ID Card · NEXUS Card
A Mexican residence card (tarjeta de residente) is not on the USPS list.
Proof of address — any one of these:
U.S. Driver's or Nondriver's ID Card · Current Lease · Mortgage or Deed of Trust · Home or Vehicle Insurance Policy
· Vehicle Registration Card · Voter Card
USPS does not accept utility bills, bank statements, or carta de residencia.
I live in San Miguel. Which address do I use?
Either — your U.S. address or your San Miguel address. Decide which one you want on the form, then provide a document showing that same address. If you're using your San Miguel address, your lease (contrato de arrendamiento) or your escritura or predial works. Documents in Spanish are fine; we handle the English details.
Can I use the same document twice?
No. If your driver's license is your photo ID, you'll need something different for your address.
Do I need to come into the office?
No. The whole process can be done remotely — you send us photos of your documents, and we do a short video call for the signature. You're welcome to come in if you'd rather.
Do I need a notary?
Not usually. USPS allows us to witness your signature over a live video call, which is what most of our members do. If a video call doesn't work for you, you can arrange an online notary instead.
Why is this required?
USPS updated its rules for Commercial Mail Receiving Agencies. Every registered mail service in the country now enters its customers' Form 1583 records into a USPS database. La Conexión is a registered CMRA — we've been serving San Miguel for over 30 years and we follow these requirements fully.
